docs: add v5.0 platform design spec

- Writing Studio (manuscript manager, sprints, snapshots, proofreading)
- AI Assistant (multi-provider: Ollama, LMStudio, GGUF+GPU, cloud APIs)
- Collaboration (git-based async, comments, review requests)
